Introduction
PLT Health and life (“PLT,” “we,” “us,” or “our”) cares about how your personal information is collected, used, and shared across our websites, mobile applications, and services (collectively, the “Sites and Services”). We maintain administrative, technical, and physical safeguards and review our security controls regularly in accordance with applicable federal and state laws.

“Personal information” means nonpublic information relating to an identified or identifiable individual. This may also be called personally identifiable information, sensitive personal data, or protected health information under applicable law. De-identified or aggregated data is not considered personal information.
Incorporation by Reference. This Policy is incorporated into our Terms of Service.
Electronic Records and Consent. We may use electronic consent-capture technologies to create an independent record of your consent, including your prior express written consent under the Telephone Consumer Protection Act (TCPA) and the federal E-Sign Act.
Your Consent
By using the Sites and Services and by submitting information to PLT or our appointed agents, you consent to this Policy and to the processing and transfer of your information as permitted by law and as described here.
By providing contact information, you agree that we may contact you directly or through service providers via email and, if you opt in, by phone calls (live or automated), prerecorded messages, and texts at the number you provide, even if on a Do-Not-Call list. You can opt out by following instructions in any message or by contacting us. Opt-outs for calls/texts apply to the specific sender unless otherwise required by law.
We may disclose information as required or permitted by law, including to protect rights and safety, comply with legal process, or pursue remedies.
Health information. Medical or health-related information is shared only to perform business, professional, or insurance functions you authorize. Vendors must protect such data under contract.
Information We Collect
We may collect the following categories within the last 12 months, from you, your authorized representative, third parties, our service providers, and through our Sites and Services:
Identifiers and contact data: name, alias, postal address, email, phone, IP address, device IDs, online identifiers.
Customer records (Cal. Civ. Code §1798.80(e)): signature, SSN (if needed for underwriting), policy and claim information, payment details.
Protected classifications: e.g., age or gender where permitted and relevant.
Commercial information: quotes requested, products or services purchased or considered, transaction history.
Account credentials: username, passwords, security questions.
Support communications: content you send us via forms, email, chat, or phone.
Internet/network activity: browsing data, pages viewed, referring URLs, ad interactions.
Geolocation data: approximate location from IP or device settings.
Audio/visual data: call recordings and website interaction recordings where permitted.
Professional/employment data: if relevant for underwriting or eligibility.
Inferences: profiles or segments derived from the above.
Sensitive personal information: e.g., SSN, precise geolocation, health data where necessary and permitted.
Analytics data: including via Google Analytics or similar tools (you may use Google’s opt-out add-on).
We retain personal information for as long as reasonably necessary for the purposes described and to meet legal obligations.
Health Information and HIPAA
If you apply for insurance through our Sites, we may collect information that constitutes Protected Health Information (PHI) under HIPAA. We use and disclose PHI only as permitted by HIPAA, applicable law, and insurance-carrier privacy practices. If this Policy conflicts with HIPAA or carrier obligations, HIPAA and carrier obligations control.

How We Protect Information
We employ layered safeguards, access controls, encryption in transit where appropriate, monitoring, and periodic assessments. No system is 100% secure. Use of the Sites is at your own risk.
How We Share Information
Affiliates. We may share with affiliates for operations, compliance, and marketing consistent with this Policy.
Network partners. With your inquiry or application, we may share with licensed agents, brokers, carriers, lead partners, or other insurance service providers to deliver quotes or services you request. Those parties may retain and use your information per their policies. Contact them directly about their practices.
Service providers. E.g., hosting, cloud, payments, analytics, call center, security, IT, auditing, and professional advisors.
Advertising and marketing partners. We may enable cookies, pixels, and similar tools for measurement or interest-based advertising.
Social platforms. If you interact with us there, the platform may collect information per its own policy.
Government, regulators, law enforcement. As required by law or valid process.
Corporate transactions. In connection with a merger, acquisition, financing, asset sale, reorganization, or bankruptcy.
Compliance and licensing. As needed to meet state/federal licensing or regulatory requests.
“Sale” or “Sharing” for Targeted Advertising
Under some state laws, certain disclosures for advertising or analytics may be deemed a “sale” or “sharing.” You may opt out (see State-Specific Rights and Your Choices).
Additional Practices
Aggregate/De-identified data. We may create and use de-identified or aggregated data and share analyses that do not identify individuals.
Data about others. If you provide another person’s contact information, you confirm you have their consent. Do not submit info about minors without lawful authority.
Cross-border transfers. Data may be processed in the U.S. and other jurisdictions with different laws. We apply required safeguards.
Children. We do not knowingly collect personal information from children under 13. If we learn we have, we will delete it.
Cookies and Tracking
We and partners use cookies and similar technologies to operate the Sites, remember preferences, perform analytics, and deliver relevant ads. You can control cookies in your browser. Blocking cookies may impair some features.
Do Not Track. Browsers may send DNT signals; we do not respond to them.
Non-U.S. Users
Our Sites are intended for U.S. users. If you access from outside the U.S., you consent to processing in the U.S.
Your Choices
Marketing emails. Unsubscribe via email footer or contact us.
Calls/texts. Follow opt-out instructions in messages or contact us.
Ads and analytics. Use platform or browser controls and industry opt-outs where available. You can also contact us to exercise state rights described below.
SMS Policy Statement
We will not share your opt-in to an SMS campaign with any third party for purposes unrelated to providing you with the services of that campaign. We may share your Personal Data, including your SMS opt-in or consent status, with third parties that help us provide our messaging services, including but not limited to platform providers, phone companies, and any other vendors who assist us in the delivery of text messages.
All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
State-Specific Consumer Privacy Rights
Depending on where you reside (e.g., California, Colorado, Connecticut, Utah, Virginia), you may have some or all of the following rights regarding personal information:
Access: Know whether we process your data and access it.
Portability: Receive certain data in a portable format.
Deletion: Request deletion of certain data we collected.
Correction: Request correction of inaccuracies.
Opt-out: Opt out of (i) sale of personal data, (ii) targeted advertising, and (iii) certain profiling with legal or similarly significant effects.
Appeal (CO/CT/VA): Appeal our decision if we decline your request.
How to Submit a Request
Email privacy@plthealthandlife.com or call 850-795-4493. Only you or your authorized agent may submit a request. We will verify identity and authority before acting. Some exemptions apply. We aim to respond within 45 days, or as allowed by law.
Authorized Agents
If you use an agent, we may require proof of authorization and identity verification. Parents/guardians may act for minors as allowed by law.
California-Specific Disclosures (CCPA/CPRA)
We collect the categories listed in Section 3.
We may disclose those categories to affiliates, service providers, network partners, marketing and analytics partners, regulators, and as otherwise described.
We do not discriminate for exercising CCPA rights.

TCPA Consent and Communications
If you provide a phone number and opt in, you agree we and our partners may contact you via live or automated calls, prerecorded messages, and texts regarding quotes, enrollment, and services. Message/data rates may apply. You can revoke consent as described in Your Choices.
Retention
We retain information as needed for the purposes described, to comply with law, resolve disputes, and enforce agreements.
Changes to This Policy
We may update this Policy. The “Effective Date” indicates the latest revision. Your continued use of the Sites after changes means you accept the updated Policy.
